PARTICIPATION REQUIREMENTS:
TAX PRIVELEGE LICENSE
All vendors selling items onsite are required to carry and provide proof of current Tax Privilege License for the City of Phoenix.
INSURANCE CERTIFICATE:
We will require an insurance certificate showing a minimum of 1 Million Liability Coverage with the following listed as additionally insured:

PAYMENT IS DUE AT THE TIME OF REGISTRATION. APPLICATIONS WITHOUT PAYMENT SUBMISSION WILL NOT BE ACCEPTED.
